    A method and apparatus for determining an optimized radiation beam arrangement for applying radiation to a tumor target volume while minimizing radiation of a structure volume in a patient, comprising: using a computer to computationally obtain a proposed radiation beam arrangement; using the computer to computationally change the proposed radiation beam arrangement iteratively, incorporating a cost function at each iteration to approach correspondence of a CDVH associated with the proposed radiation beam arrangement to a CDVH associated with a pre-determined desired dose prescription; comparing the dose distribution to a prescribed dose for the tumor volume and surrounding tissue structures, and increasing or decreasing radiation beam intensity if the change of the proposed beam arrangement leads to a greater correspondence to the desired dose prescription to obtain an optimized radiation beam arrangement.

    An apparatus and method are disclosed for displaying a path between a selected target and selected points on a patients skull and for guiding surgical instruments along any selected path. The system is comprised of an image display system, an articulated arm and probe, and a stereotactic system. The sub-systems are coupled to one another so that the articulated probe may be used to select patient fiducial points that correspond to selected image fiducial points. Using these points, the image display system coregisters the external locations to the displayed images so that the probe condition may be displayed with the displayed images. The system further permits the identification of a selected target within a patients brain and to project a path from the external position to the target prior to the performance of a craniotomy. After evaluation of the path, a surgeon may lock the stereotactic system in place to preserve a selected surgical path and to guide instruments along that path. A method of utilizing the system to perform such surgical procedures is also described.

    A small tabletop stationary five-degree of freedom device such as a “robot” is used to define the treatment region by tracing the region under direct visualization and then to precisely deliver the treatment plan created by an automatic planning system by positioning a single low energy radiation source, or a plurality of low energy sources connected to each other in a predetermined parallel or similar geometry, each source equipped with blocking and attenuation mechanisms, at a plurality of positions in a planar fashion across or through a selected treatment field, thereby delivering a plurality of parallel overlapping beams indexed on a millimeter or submillimeter grid such that a concentration of dose is achieved at a variable depth in tissue relative to the dose where the radiation first enters the tissue and can be used to treat regions on or below the surface of tissue, in a cavity and underlying region created following a surgical resection, on or below the surface of an internal cavity, hollow viscus, or lumen, or deep in tissue adjacent to an inserted probe or conduit or catheter. By generating a plurality of overlapping beams indexed on a millimeter or submillimeter grid that converge on a target volume loaded with gold nanoparticles, a tumorcidal dose of radiation can be delivered in as little as a single session to tumor cells but not to normal cells within or outside the treatment volume. This approach also makes it possible to deliver serial radiosurgical treatments.

    Brachytherapy treatment of a patient's lung tissue following resection is effected using a balloon applicator which is inserted, normally through the same opening used for the surgery, through the chest wall and into the cavity. The lung and chest openings are closed around the applicator and generally sealed around the applicator. A suction port is provided, in a suction circuit of the applicator, to withdraw fluid from the pleural cavity, at intervals as needed, to assure that the lung can be inflated. Different embodiments of suction circuits are disclosed. A bronchial applicator and method are also disclosed.
